Case Management

Individuals served by The Arc of Ventura County are assigned a case manager. The case manager’s role is to coordinate participant services at The Arc of Ventura County . The case manager will also collaborate with the participant’s circle of support to assist in identifying and accessing other services in the community the individual may need or desire.

The case manager is available to meet with the individual and discuss his/her progress, review goals and plans to meet them, provide information and assist with informed decision making, resolve conflicts to the participant’s satisfaction, and to serve as their advocate.

The case manager will collaborate with others to support the individuals they serve in the process of developing meaningful lives and the resources to support their choices.
