CASE MANAGER/ASSOC. CASE MANAGER

Immediate opening in Simi Valley- Cochran

Conducts formal and informal assessments to determine the needs, preferences and capabilities of people with developmental disabilities.  Facilitates the development of person-centered plans with participants and all support team members addressing living arrangements, education, work, community involvement, recreation and social relationships.  Develops goals and specific outcomes desired based on participants’ needs, preferences and interests.  Periodically reviews achievements and recommends modifications.  Researches, develops and maintains information on community and other resources and supports relevant to the needs of participants.

COMMUNITY INTEGRATION -- INSTRUCTORS

Immediate openings in Camarillo,Simi Valley

Encourage and support people with developmental disabilities to lead self-directed lives and to participate fully in our community.  Implement and augment person-centered plans toward a life of opportunity, well-being, freedom and contribution.  Assist people in attaining their goals and making everyday choices around community access and inclusion, personal finances, physical well-being, relationships, employment and recreation.  Position offers opportunities to share personal talents. Requires creativity, excellent communication and a sincere commitment to improving the lives of people with disabilities.  Starting $10.50/hour, DOE.

COMMUNITY INTEGRATION – JOB COACH

Immediate openings in Simi Valley

Encourage and support people with developmental disabilities to achieve their employment goals and objectives.  Support takes place during job training, in group and individual employment settings.  Ensures work requirements and quality standards are met.  Emphasizes safe work practices.  Acts as liaison with community employers, ensuring positive work relationships.    Monitors and documents individual progress and appropriateness of job placement.  Starting $10.50/hour, DOE.

COMMUNITY LIVING – DIRECT SUPPORT PROFESSIONAL

Immediate openings in Ojai

Provide direct support and service to six adults sharing a home.  Implement and augment person-centered plans ensuring the health and well being of residents.  Encourage and support people to lead self-directed lives, assisting with homemaking, personal care and self-administration of medications; provide opportunities for social integration and recreation; identify and resolve problems and barriers; document progress and observations; and provide feedback to residents and their circles of support.  CNA or nurse aide experience is helpful but not required.  Position requires creativity, excellent communication and a sincere commitment to improving the lives of people with disabilities. Starting $10.50/hour, DOE.
